Job summaryDerbyshire Dales Primary Care Network are seeking a motivated and compassionate Physician
Associate (PA) to join our dedicated Home Visiting Service. Working
as part of a multidisciplinary primary care team, you will provide high-quality
clinical assessment, diagnosis, management, and follow-up care to patients
within their own homes.This is an exciting opportunity for a skilled
clinician who enjoys autonomous working while remaining supported by
experienced GPs and other healthcare professionals. You will play a pivotal
role in improving access to care, reducing unnecessary hospital admissions, and

Qualifications
EssentialMSc or Postgraduate Diploma in Physician Associate Studies or equivalent recognised qualification.Successful completion of the Physician Associate National Examination.Registration with the General Medical Council as a Physician Associate. Experience working in primary care, urgent care, community care, or a similar clinical setting.Experience undertaking patient assessments and formulating management plans.Experience working within a multidisciplinary healthcare team.Knowledge & SkillsExcellent clinical assessment and diagnostic skills.Ability to recognise and manage acute and chronic presentations within scope of practice.Strong communication and interpersonal skills.Ability to work independently whilst recognising limitations and seeking support appropriately.Competent IT skills and experience using electronic patient record systems.Understanding of safeguarding responsibilities for adults and children.Good organisational and time-management skills.Personal AttributesProfessional, compassionate, and patient-centred approach.Flexible and adaptable to changing service demands.Commitment to continuous professional development.Ability to travel independently throughout the service area.DesirableQualificationsAdditional training or qualifications in urgent care, frailty, chronic disease management, or community care.ExperiencePrevious experience within a Home Visiting Service.Experience in General Practice.Experience managing elderly, frail, or housebound patients.Experience conducting care home visits.Participation in clinical audit, service development, or quality improvement projects.Knowledge and SkillsKnowledge of community services and local healthcare pathways.Experience using EMIS, SystmOne, or similar clinical systems.Understanding of population health management and preventative care.Experience supporting trainee clinicians or students.Personal AttributesAbility to build effective relationships across organisations.Strong problem solving and decision making skills.Enthusiasm for service innovation and integrated working.
