Physiotherapist - Home-Based!

We are currently recruiting for an experienced and dedicated healthcare professional to join a busy Intermediate Care service. The successful candidate must: Be a car driver with access to a vehicle for community-based visits and service delivery Hold current HCPC registration Have experience working within a Multidisciplinary Team (MDT) environment Have previous rehabilitation experience, which would be highly advantageous Possess post-graduate clinical experience, as the service manages complex caseloads and requires confident clinical decision-making This role involves working across all Intermediate Care pathways, including: Reablement services Rapid Response pathways Rehabilitation beds and community rehabilitation services Candidates must also demonstrate previous community experience and the ability to work independently within patients’ homes and community settings while delivering high-quality, patient-centered care. The ideal applicant will be adaptable, organized, and capable of managing complex patient needs within fast-paced healthcare environments. Strong communication, assessment, rehabilitation, and care-planning skills are essential to support patient recovery, promote independence, and prevent unnecessary hospital admissions.
