What you will do:

• Develop and implement care plans for young adults transitioning to independence.

• Provide line management support, overseeing the performance and development of junior staff.

• Apply knowledge of legislation including the Mental Capacity Act 2005 and the Care Act 2014.

• Manage budgets effectively to support service users.

• Liaise with professionals across health and social care to coordinate holistic services.

• Apply safeguarding procedures to protect vulnerable adults and children.

Requirements:

• Willingness to undertake line management responsibilities

• Submission of a fully completed Adult MOC form

• Valid driving licence
