About the Role
Make a difference in people’s lives while shaping how we deliver health and quality in supported living.
We’re looking for a qualified nurse with a passion for improving health outcomes for people with learning disabilities and complex needs. You’ll combine hands-on nursing expertise with a focus on quality, medication safety, and person-centred practice.
What you’ll be doing:
Leading on complex health support and ensuring personalised support plans are accurate and up to date.
Building strong links with hospitals, GPs, and specialists, making sure people have positive healthcare experiences.
Overseeing medication audits, spot checks, and staff competency assessments.
Promoting health and wellbeing initiatives, including diet, nutrition, hydration, mobility, and epilepsy care.
Monitoring and acting on changes in physical and mental health needs.
Using data and audits to drive continuous improvement in health quality.
What we’re looking for:
NMC-registered nurse (RGN, RMN, RNLD).
Experience supporting people with complex health needs.
Strong understanding of medication management and audits.
Confident working with multidisciplinary teams and external health professionals.
Passionate about person-centred care and empowering people to manage their own health.
