Social Worker Adults - Complex needs

We are currently recruiting for an experienced Social Worker to join the Complex Care Team in Runcorn to support with a service backlog and ongoing case management. This is an excellent opportunity for a qualified and confident practitioner who has successfully completed their ASYE programme and has experience working within adult social care and complex case management. Key Responsibilities Completing Care Act Assessments Undertaking outcome-focused reviews Carrying out Mental Capacity Assessments and Best Interest Decisions Supporting with safeguarding enquiries under Section 42 Completing CHC checklists and attending DST meetings Participating in duty rota responsibilities Working collaboratively within a multi-disciplinary team (MDT) approach alongside internal services and external partner agencies Requirements Qualified Social Worker with Social Work England registration Completion of ASYE programme Previous experience within Adult Social Care / Complex Care Strong knowledge of the Care Act, Mental Capacity Act, and safeguarding legislation Ability to work independently and manage a varied caseloadConnect2Halton is a trading style of Halton & Kent Commercial Services LLP- A joint venture between Halton Borough Council & Commercial Services Kent Ltd.
