About the role
Qualifications & Professional Registration
* Registered Nurse (Adult or equivalent) with current NMC registration and a Master’s degree in Advanced Clinical Practice (or equivalent ACP‑mapped qualification).
* Practising as a recognised Advanced Clinical Practitioner or working at advanced level, with V300 prescribing qualification (or actively working towards) and clear evidence of ongoing CPD.
* Desirable: ACP credentialing/portfolio aligned to the NHS England ACP framework and a teaching, mentorship, or Practice Assessor/Supervisor qualification.
Clinical Experience
* Extensive post‑registration experience delivering autonomous, advanced clinical care to people with complex physical health needs in community and home‑based settings.
* Strong background in managing long‑term conditions, frailty, multi‑morbidity, and undertaking advanced clinical decision‑making, risk assessment, and escalation in non‑acute environments.
* Proven ability to work collaboratively across primary care, community services, secondary care, and social care, including experience in hospital at home, rapid response, virtual wards, or admission avoidance, and supporting people with neurological conditions, complex disability, or end‑of‑life needs.
Clinical Skills & Competencies
* Advanced holistic assessment, diagnostic reasoning, and treatment planning, including the development and review of complex, person‑centred care plans.
* Safe and effective prescribing and medicines optimisation, high‑quality clinical documentation, and strong application of safeguarding principles within community settings.
* Able to practise independently and accountably, maintaining professional boundaries, with advanced clinical skills relevant to service need (e.g. IV therapy, complex wound care, catheter care) and experience contributing to pathway development or service innovation.
Education & Leadership
* Actively supports workforce development through clinical supervision, mentorship, coaching, and teaching, with a strong commitment to evidence‑based practice.
* Highly effective communicator, able to explain complex clinical information clearly to patients, carers, and multidisciplinary colleagues.
* Desirable: Practice Assessor and/or Practice Supervisor qualification.
Research, Audit & Quality Improvement
* Demonstrates the ability to critically appraise evidence and apply research findings to clinical practice.
* Experience contributing to or leading audit, service evaluation, and quality improvement initiatives, with a clear commitment to reflective practice and continuous improvement.
* Involvement in research activity, guideline development, or outcomes measurement
