key role in medicines optimisation, structured medication reviews, long-term
condition management, prescribing support, and medicines safety.The post holder will work within their clinical competencies to provide specialist pharmaceutical support to the practice including working with members of the wider healthcare team to improve care and support safe and effective prescribing.A strong background in primary care is essential, along
with excellent communication, organisational, and clinical decision-making
skills. Experience with SystmOne is desirable. Independent Prescriber
qualification is advantageous, although not essential.Main duties of the jobTo reconcile medicines following discharge from hospitals or intermediate care and working with patients and community pharmacists to ensure patients receive the medicines they need post-discharge and reduce of readmission.To manage repeat prescriptions requests, resolving queries where possible within scope of practice.
