The role

As part of the Pathway 3 service, you'll support safe and timely hospital discharge for people with more complex care needs. You'll work closely with patients, families, health colleagues and community providers to:

• Complete strengths-based assessments and care planning
• Coordinate discharge arrangements and onward community support
• Manage safeguarding and risk appropriately
• Help prevent delayed discharges while ensuring safe outcomes
